Elder Care and Transitions Consulting, LLC is a geriatric care management company providing support and assistance to seniors, their families, and caregivers during times of transition in care requirements. 

• We serve as medical advocates and care managers for seniors with distant caregivers.  This typically includes:
• Scheduling and attending doctors visits
• Coordinating medical care among multiple physicians
• Setting up medications and coordinating prescriptions and pharmacy communications
• Serving as an advocate and liaison with care facility staffs and local support providers and organizations.
• We conduct geriatric assessments of seniors to help families, trust companies, elder law attorneys, and home health agencies identify the current medical, physical, and mental status of the senior, including a recommended course of action.
• We help families locate, screen, and select top-quality home health services and care facilities, minimizing the time it takes the family to locate these services and increasing their confidence and peace of mind with the decisions they make. 
• We provide seniors and families free referrals to professional service providers, including elder law attorneys, financial planners, transportation companies, and relocation specialists.

We provide unbiased, objective information about the full range of available care options and do not accept referral fees from any of the care providers we present for consideration. Our first concern is the best interests of the care recipient, striving to help them live as independently as possible, consistent with their safety and comfort.
