Elder Care and Transitions Consulting, LLC is a geriatric care consulting and management company providing support and assistance to seniors, their families, and caregivers during times of transition in care requirements. 

• We help families locate, screen, and select top-quality care services, minimizing the time it takes the family to locate these services and increasing their confidence and peace of mind with the decisions they make. 
• We also advocate for care recipients already in a care facility by periodically reviewing the ownership, operations and staffing of the facility and the adaptation of the care recipient to insure that care continues at the levels expected.  We then send the family a report.
• We provide seniors and families free referrals to professional service providers, including elder law attorneys, financial planners, transportation companies, and relocation specialists.

We provide unbiased, objective information about the full range of available care options and do not accept referral fees from any of the care providers we present for consideration. Our first concern is the best interests of the care recipient, striving to help them live as independently as possible, consistent with their safety and comfort.
